Сфера охорони здоров’я як об’єкт публічного управління

Abstract

У статті з’ясовано, що публічне управління у сфері охорони здоров’я – публічно владна діяльність відповідних суб’єктів адміністрування на різних рівнях, яка не може розвиватись та функціонувати ізольовано від проблем сьогодення. Публічне управління у медичній сфері здійснюється за допомогою наступних складників – форми організації системи охорони здоров’я; методу управлінського впливу; джерел фінансування. Складовим елементом загального управління, який зумовлює наслідок роботи його об’єкта, є механізм управління. Визначено основні форми, методи та функції суб’єктів публічного управління сферою охорони здоров’я. Констатовано, що основним завданням публічного управління медичною сферою є розбудова орієнтованої на пацієнта системи, здатної забезпечити справедливе загальне охоплення послугами охорони здоров’я всього населення країни. Проаналізовано суб’єктів публічного управління загальнодержавного та місцевого значення; суб’єктів, наділених спеціалізованими публічними повноваженнями. Визначено основні завдання суб’єктів публічного управління у сфері охорони здоров’я. З’ясовано ключові завдання консультативно-дорадчих органів, які також віднесено до суб’єктів публічного управління у сфері охорони здоров’я (Колегія МОЗ України, Клініко-експертна комісія МОЗ України, Медична рада закладу охорони здоров’я України, Національна рада з питань протидії туберкульозу та ВІЛ-інфекції/СНІДу). Проаналізовано повноваження профільного парламентського Комітету з питань здоров’я нації, медичної допомоги та медичного страхування. Окреслено проблематику даного напряму діяльності та запропоновано шляхи їх вирішення. Теоретично обґрунтовано, що для досягнення якісного результату система управлінських механізмів на всіх рівнях публічного управління сферою медицини повинна діяти узгоджено, цілісно та збалансовано. Акцентовано увагу на необхідності подальшої автономізації закладів охорони здоров’я та децентралізованому підході до ухвалення управлінських рішень, що, як наслідок, сприятиме покращенню ефективності захисту прав пацієнтів під час реалізації їх конституційного права на охорону здоров’я. The article clarifies that public administration in the field of health care is a publicly powerful activity of the relevant administration subjects at different levels, which cannot develop and function in isolation from today’s problems. Public management in the medical field is carried out with the help of the following components – the form of organization of the health care system; management influence method; sources of funding. A constituent element of general management, which determines the outcome of the operation of its object, is the management mechanism. The main forms, methods, and functions of subjects of public management in the field of health care are determined. It has been established that the main task of public management of the medical sphere is the development of a patient-oriented system capable of ensuring fair general coverage of health care services for the entire population of the country. Subjects of public administration of national and local significance were analyzed; entities endowed with specialized public powers. The main tasks of public administration subjects in the field of health care are defined. The key tasks of consultative and advisory bodies, which are also attributed to subjects of public administration in the field of health care (College of the Ministry of Health of Ukraine, Clinical Expert Commission of the Ministry of Health of Ukraine, Medical Council of the Health Care Institution of Ukraine, National Council on against tuberculosis and HIV/AIDS). The powers of the specialized parliamentary Committee on the Health of the Nation, Medical Care and Health Insurance were analyzed. The problems of this field of activity are outlined and ways of solving them are proposed. It is theoretically justified that in order to achieve a quality result, the system of management mechanisms at all levels of public management in the field of medicine should act in a coordinated, integral and balanced manner. Attention is focused on the need for further autonomy of health care institutions and a decentralized approach to management decision-making, which, as a result, will contribute to improving the effectiveness of the protection of patients’ rights during the implementation of their constitutional right to health care.
