Стоматологічний статус та одонтофобічні реакції у дітей з різним рівнем психічного здоров’я

Abstract

Мета дослідження: підвищення якості стоматологічного лікування дітей з різним рівнем психічного здоров’я шляхом визначення зв'язку їх стоматологічного статусу із медико-психологічними особливостями. У дисертаційній роботі наведені механізми підвищення якості стоматологічного лікування дітей з різним рівнем психічного здоров’я (РРПЗ) шляхом визначення зв'язка стоматологічного статусу з їх медико-психологічним станом та корекції одонтофобічних реакцій (ОФР). ОФР є однією з провідних проблем стоматології, оскільки вони є широко розповсюдженими та значно ускладнюють, а часто навіть унеможливлюють адекватне проведення огляду ротової порожнини та стоматологічних процедур. Діти внаслідок анатомо-фізіологічних особливостей нервової системи (переважання процесів збудження над процесами гальмування не дозволяє проводити адекватне конативне регулювання біхевіоральних актів, а превалювання конкретно-образного мислення над абстрактно-логічним поряд із браком фактичних знань не дозволяє у повній мірі осмислити ситуацію та підібрати конгруентний модус реагування) більш схильні до фобічного реагування у ситуації стоматологічного огляду, що є стресовим для них. The dissertation identifies the relationship between dental status and medical-psychological characteristics in children with different levels of mental health (DLMH) and describes congruent measures for the prevention and correction of odontophobic reactions (OFR) by increasing compliance in the child-parent-dentist chain. OFR is one of the leading problems in dentistry, as they are widespread and significantly complicate, and often even make it impossible to adequately conduct an examination of the oral cavity and dental procedures. Children due to the anatomical and physiological features of the nervous system (the predominance of excitation processes over inhibition processes does not allow for adequate conative regulation of behavioral acts, and the prevalence of concrete-figurative thinking over abstract-logical along with lack of factual knowledge does not allow to fully comprehend the situation response) are more prone to a phobic response in a dental examination situation, which is stressful for them. The presence of mental illness in a child creates a psychopathological basis for an anxious response to a new, unusual situation, and thus increases the risk of developing OFR in such a child, as well as significantly complicates the dental examination due to difficulties in achieving compliance with a child.The study is based on the results of a comprehensive examination (anamnestic, dental, clinical and psychopathological, psychoexperimental, catamnestic, ethological, clinical and statistical methods), 100 patients were examined, including 30 patients with autism, from group 1 (G1), 30 patients with oligophrenia from group 2 (G2), 20 patients with ADHD from group 3 (G3) and 20 mentally healthy children from group 4 (G4) with various dental pathologies.
