Клініко-лабораторні особливості та діагностична значимість імунологічних змін в прогнозуванні перебігу кору у дорослих

Abstract

З метою підвищення ефективності прогнозування перебігу кору у дорослих на підставі з’ясування клініко-лабораторних особливостей та діагностичної значущості імунологічних змін було проведено комплексне обстеження 175 дорослих хворих на кір, які лікувалися в Комунальній установі «Обласна інфекційна клінічна лікарня» у сезонності 2017-2019 р.р. Спеціальні дослідження включали визначення IgM та IgG до вірусу кору, вмісту IFN-γ, TNF-α в сироватці крові методом ІФА на базі Навчально-лабораторного центру Запорізького державного медичного університету. Серед включених в дослідження хворих на кір було 94 жінки та 81 чоловік. Вік хворих був від 18 до 58 років. При постановці діагнозу кір користувалися критеріями ВООЗ (2013): у 32,6 % хворих випадок кору був лабораторно підтверджений наявністю позитивного результату дослідження IgM проти вірусу кору, у 24,6 % пацієнтів кір діагностовано як епідеміологічно пов’язаний випадок кору, у 42,8 % пацієнтів діагноз кору відповідав критеріям клінічно сумісного діагнозу. Усі хворі включені в дослідження за випадковою ознакою та інформованою письмовою згодою. In order to improve the effectiveness of predicting the course of measles in adults based on the clarification of clinical and laboratory features and the diagnostic significance of immunological changes, a comprehensive examination of 175 adult measles patients who were treated at the Municipal Institution "Regional Infectious Diseases Clinical Hospital" during 2017-2019 seasonality. Special studies included the determination of IgM to the measles virus, IgG to the measles virus, the content of IFN-γ, TNF-α in blood serum by the ELISA method on the basis of the Training and Laboratory Center of Zaporizhzhia State Medical University. Among the measles patients included in the research, there were 94 women and 81 men. The age of the patients was from 18 to 58 years. WHO (2013) criteria were used for the diagnosis of measles, according to which in 32.6 % patients, the case of measles was laboratory confirmed by the presence of a positive result of IgM against the measles virus, in 24.6 % patients with measles was diagnosed as an epidemiologically related case of measles, in 42.8 % patients the diagnosis of measles respond the criteria of a clinically compatible diagnosis. All patients were included in the research on a random basis and informed written consent.
